CIF Student-Athlete Code of Conduct
This Code applies to all student-athletes involved in interscholastic sports in California, and requires that participating students demonstrate high standards of ethics and sportsmanship and promote the development of good character and other important life skills. The highest potential of sports is achieved when participants are committed to pursuing victory with honor according to six core principles: trustworthiness, respect, responsibility, fairness, caring, and good citizenship (the “Six Pillars of Charactersm”).
Health Statement Form
A Student Accident Insurance policy is available should you not have the required coverage. This is an excess coverage plan that provides all of the benefits that are required by law. In addition it provides certain benefits that are not usually covered by many medical insurance plans, such as dental costs, emergency medical care, and ambulance service. If you plan to purchase the Student Accident Insurance please have your student bring the envelope (stamped and ready to mail with your check inside) to the coach so that your purchase can be recorded and mailed.Please see to it that these Health Statement / Insurance Verification and Parent Consent cards are completed by you and your physician. Have your student return both cards to the coach.
Preparticipation Physical Evaluation
All students participating in interscholastic sports under the supervision of the Oakland Unified School District are required to have a physical examination each year by a physician licensed to practice in California. It is necessary for you to have your student examined by a physician of your choice. Please fill in “Part A” (Page 1) of this Preparticipation Physical Evaluation form and have your examining physician fill in “Part B” (Page 2). Return the completed form to your student’s coach.

Shadow Day Forms

Site Visit Form – Host
The host form is for the Oakland Tech host student to fill out. The host student must have his or her teachers sign it, before the Shadow Day. Please give the teachers some reasonable amount of warning (in case there's a test or something). A couple of days to a week is fine. You do not have to get the principal's office to sign before the shadow date, but you can if you want.
Site Visit Form – Visitor
The visitor form is for the visiting student to fill out; bring it to the visit date and turn it in at the office. (The host student does not fill out anything on this form.)

Student Forms

Request for Peer Tutor
Peer Tutors are available on a first-come, first-served basis. Their purpose is to provide direct instructional support for students, especially to those who are not doing well in class.Peer Tutors can: Work one-on-one with a student Work with small groups Take a student or a small group to the library to do a project or to study Circle the room helping students Help students at the computer
Request for Senior Mentor
If you know of a 9th grader who could benefit from working with a senior mentor and who wants to work with one, please give him/her an application to complete, and submit to Ms. Dargahi.
